Get your money and get out: a collaboration to facilitate discharges from state psychiatric hospitals by integrating HCH SOAR services

This poster will present a model where a Health Care for the Homeless disability benefits specialist is placed within a state psychiatric hospital to implement the SOAR (SSI/SSDI Outreach, Access, and Recovery) model, a SAMHSA promising practice, within the hospital’s multidisciplinary team to expedite discharges.
