Integrated Care Anywhere: Utilizing Interdisciplinary Visits and Shared Decision- Making to Engage Reticent Consumers

This workshop will focus on integrated care co-visits and shared decision-making among four entities: the consumer, the behavioral health care provider, the consumer health navigator and the primary care provider. Integrated care visits can be an important tool in reducing barriers to consumer healthcare access, improving communication, decreasing medication errors, and drawing upon the strengths of existing consumer-provider relationships. This model can be translated to a variety of care settings and is a particularly effective tool to engage consumers with more profound behavioral health barriers who have been reticent to engage in healthcare services in the past.
