What is an Ideal Health System Partner? A Health System's Journey to Create and Implement a Healthcare for the Homeless Scorecard

People experiencing homelessness have high burdens of illness and injury and can be expensive frequent users of hospital resources. A health system seeks to understand and encourage local hospital involvement. In partnership with NHCHC and other HCH leaders, a scorecard was created to guide hospitals through a comprehensive assessment of their current commitment to this population, and identify areas of improvement. The presentation will focus on the development, the implementation, and the outcomes of the scorecard. A review of best practices, how to spread innovation across a health system, experiences, and constructive solutions will be shared.
