Best Practices for Shelter Based Care

Wednesday, May 16, 2018: 12:30 PM-1:30 PM
Regency Room (Hyatt Regency Minneapolis)
HCH providers have a specialized knowledge in providing services to persons experiencing homelessness. One way we break down barriers and increase access care is by providing coordinated care in the shelter. But this can be a tricky process; at times the busiest and most crowded shelters can have the poorest productivity. And others have more persons accessing services than the system can handle. This session will tap into the expertise of our community by facilitating a discussion to get answers to the following question: “What are the best practices for coordinated shelter care?"
Speakers:
Annie m Nicol, FNP (Director of Homeless Services, Petaluma Health Center)
Mary L. Tornabene, FNP (Family Nurse Practitioner, Heartland Health Outreach)
