Mapping the Road to Healthcare for Vulnerable Populations

Two health centers from Wisconsin and Tennessee both work to engage individuals in a holistic manner through various outreach techniques in order to address barriers to care. For the vulnerable populations served by these health centers, key barriers include transportation, communication, trauma, and past experiences. Through outreach, we establish a relationship with potential patients and educate them on health services offered. This is a crucial step in mapping the road to a patient-centered healthcare “home,” where team members are mindful of trauma and accommodate the needs of each individual patient. Patients play a key role on the healthcare team.
