This project was designed in collaboration with a homeless shlter organziation and healthcare. The project improved the health of the homeless population with chronic diseases, in a metropolitan suburb. The project plan was the implementation of evidence based strategies which were demonstrated to be feasible, effective, and efficient quality improvement tools for the homeless population. The impact resulted in a 50% reduction of ED visits with homeless adults who completed the program. The retention rate was 70%, evaluated to be augmented by the innovative alignment of case manager follow-up with the participants.
